A grandmother who bravely battled cancer for 30 years died after she was fed mushrooms at a North-East care home ( despite staff having been warned she was allergic to them.

A care plan drawn up by social services for Sylvia Surphlis, 77, stated clearly that she could not eat mushrooms, nuts, sweetcorn or oranges.
